Six procedures sized for a laminate pouch. Stick one inside the van door and one on the equipment it covers.
Set print scaling to 100 percent or Actual size, never Fit to page. Fit to page shrinks the sheet by a percent or two, which is the single most common reason a printed card comes out the wrong size.

Why a card and not just the app
A card on the van door needs no battery, no signal and no login, and a technician who has done the job forty times will glance at it rather than open anything. The QR code is there for the day the job is unusual and he wants the full set, including whichever version is current.
The card is the reminder. The record of who read the procedure is the part that has to be electronic, because paper cannot tell you which version someone saw.
